Host Home

A host home provides individuals with disabilities with a safe, loving, and supportive home environment. This residential program model ensures that our clients can live with a caring family/individual, in the family or individual's home. Unlike supported community living services, a host home provides our consumers with a constant family structure, instead of rotating staff. The host family, or individual, has the privilege of gaining another family member, and the consumer has the wonderful opportunity to join their family.

Supported Community Living (SCL)

Supported Community Living provides support to individuals 18 and older with intellectual disabilities in their home. Services are structured to meet each individuals own strengths and needs These services are to assist each individual with daily living skills in their home and out in the community. Our staff would be providing assistance with cleaning, cooking, hygiene, transportation, community skills, personal needs, medication administration, and daily goals tailored to each of their individual plan.

Habilitation (HAB)

Habilitative Services are designed to assist individuals with chronic mental illness to live successfully in their home and community based settings. These services promote personal independence, skills training, community integration, and other habilitation supports needed to achieve overall health and social well- being. Supports are based on the individuals's wants and needs, from intermittent services to daily 24 hour support.
